Warnings:

The major function of iron is to form part of hemoglobin, which is responsible for carrying oxygen in the bloodstream from the lungs to muscles and to the brain.

Any reduction in hemoglobin can result in chronic fatigue and low energy levels.

    Critical Nutritional Concerns

    Two major nutritional problems develop as a direct result from the Roux-en-Y gastric bypass procedure in approximately thirty seven (37%) percent of all patients:

  • Iron Deficiency (as measured by serum ferritin) with concurrent low hemoglobin and hematocrit levels.

    These debilitating signs appear as early as 5 to 6 months postoperatively.

    Why is this?

    The best source of iron, heme iron, comes from red meats.

    However, the bioavailability of heme iron is only around 10%. And that's in people who have normal, anatomically-structured digestive systems.

    Non heme iron from vegetable sources has a bioavailability of around 1%.

    Hydrochloric acid is required for heme iron absorption in the duodenum.

    Remember, in the bypass patient, the newly-constructed pouch has negligible amounts of acid secretion, plus the entire duodenum is bypassed.

    To make matters worse, many bypass patients can't tolerate red meat.

    Beef for example, has a chewy, fibrous texture. This comes from collagen, an albuminoid, which is a component in the intercellular connective tissue of meats.

    In order to break down collagen, hydrochloric acid and specifically the digestive enzyme, pepsin, are required before cellular proteins can be digested.

    Collagen, meat proteins and heme iron must be in an acidic environment, with a pH of no more than 3 for digestion and absorption to take place.

    In the bypass patient, meat slowly passes through the the stoma of the newly-created, small pouch (for some patients it takes nearly two hours or is promptly regurgitated).

    When meat enters the acid free environment of the jejunum, it often enters virtually intact. Any absorption of meat protein and heme iron is poor at best.

    Therefore, after five to six months of reduced intake of meat and the subsequent reduction of any iron from any dietary source, gastric bypass patients can begin to feel the debilitating effects of iron deficiency anemia.

    To assist in bringing iron levels back to normal, try Bari-Tec Blood Support Formula. It contains the iron amino acid chelate (ferrous bis-glycinate) patented by Albion Laboratories.

    This special iron chelate, once ingested, is absorbed intact by active transport systems in the brush borders of the epithelial cells in the jejunum.

    Many iron deficient bypass patients may require (for a few months) up to 80 mg of iron (as ferrous bis-glycinate) on a daily basis.

  • Vitamin B12 deficiency appears within two years after gastric bypass surgery.

    To deal with microcytic, normocytic and macrocytic anemias resulting from vitamin B12 deficiency and iron deficiency after gastric bypass, Bari-Tec Blood Support Formula provides 500 mcg of crystalline (unbound) vitamin B12 (methylcobalamin) and 80 mg of iron as ferrous bisglycinate.

    That's more than 80 times the RDA of vitamin B12.

    At 10,000 times the RDA, B12 still appears to be non-toxic.

    Dietary B12 in food is protein-bound. It must be cleaved away from the protein by hydrochloric acid and pepsin, both of which are secreted in the stomach.

    In the gastric bypass patient the stomach is bypassed, thereby making it all but impossible to absorb dietary B12.

    However, in numerous studies, supplemental sources of crystalline (unbound) vitamin B12 (methylcobalamin) in doses of 250 mcg or more have been effective in maintaining adequate serum levels of B12 in 83 percent of gastric bypass patients.

    Some researchers believe that intrinsic factor, a substance secreted in the stomach which protects vitamin B12 from the ravages of hydrochloric acid, is no longer necessary for the absorption of this important vitamin in the gastric bypass patient.

    Since the jejunum is an essentially acid-free environment, crystalline vitamin B12 can pass unabated to the distal portion of the ileum.

    Once there, much of this large dosage of vitamin B12 is absorbed by passive diffusion.

    After laboratory analysis shows normalization of serum iron and B12 levels, always consult with your bariatric surgeon or your bariatrician as to the amount of Bari-Tec Blood Support Formula you should be taking as a maintenance dosage.

    If you are a patient who suffers from chronic iron deficiency and low hemoglobin levels, your serum ferritin, hemoglobin and hematocrit levels should be evaluated periodically.
